A Letter from Dr.Daria Blyskal

Dear patients,
Happy New Year and Happy Anniversary!  The healthcare landscape has changed much over the past 10 years since we opened the Sayville practice.  I am deeply grateful to all of you, and particularly those of you who followed me from the East Islip office, for your enthusiasm as we endeavor to improve your primary care experience.

I will be away on leave for medical/family reasons from Saturday February 1st to Monday March 31st .  During this time, board-certified family physician Dr. Ying Qian will have my cell phone and be performing all the services you expect from my practice: in person office hours, remote contact via my cell phone # and email address, ePrescribing routine and controlled medications, ordering labs and radiology tests, reviewing results and faxes,  processing prior authorizations, etc.

The search for a permanent colleague continues, and I am hopeful that this temporary replacement will be a step in the right direction of expanding the practice, accepting new patients, etc.  Here is her picture and introduction – she looks forward to meeting you!

“I am a New York-based board-certified family medicine physician.  My medical degree and residency training were completed at Stony Brook University.  I’m a former assistant professor at Mount Sinai Hospital, where I was able to practice my clinical interests in preventive health, chronic and acute disease management, wellness, nutrition, women’s health and pediatrics.  I am passionate about developing meaningful relationships and collaborating with patients and families in order to achieve the highest level of personal wellbeing.  In my free time, I enjoy traveling, playing the piano, and spending time with family and friends.”

Here are logistics that may be relevant to you:

  • The link on my website to make in-person appointments with Dr. Ying will go live by the end of January.  I will be at the office Saturday January 25th and every weekday in January until Friday the 31st.  I return to the office on Tuesday April 1st.
  • If you need a form filled out, would like to schedule a phone call, or have other needs that do not require an in person physical exam, nothing changes.  Please text, call or email and Dr. Ying will be happy to accommodate you.
  • If you are on a daily controlled medication, nothing changes.  Please text my cell 1-2 days before your refill is due and specify which pharmacy you’d like.  If it is not available at your first choice pharmacy, locate alternate pharmacies in a driving radius acceptable to you to inquire about availability.
  • If your insurance has changed, please text or email me a pictures of the front & back of your new card.
  • If you would like to change your billing method, please give Dr. Ying the specifics and she will communicate them to me.
  • If you have a surgery planned, please ask your surgeon whether preoperative testing and medical clearance is necessary.  If yes, ask whether the hospital/surgicenter will perform the testing or Dr. Ying should order the testing.  Dr. Ying is capable of performing and interpreting EKGs in my office if requested as part of the clearance.  Please schedule an in-person appointment with her after your preoperative testing is complete and at least 1 business day before your surgery.

I will miss you and look forward to springtime together.

Sincerely,
Daria Blyskal, MD
