For Current Patients
Until at least Monday 4/6/20, please call, text or email Dr. Blyskal to schedule an appointment. She will only be examining patients for “sick/acute visits” (whether infectious or non-infectious) and regretfully not accepting new patients other than family members of current patients.
Request to make an appointment online via this link to Dr. Blyskal’s electronic medical record. You will receive an email from Patient Fusion when your appointment request is pending, when it is approved, and reminders 1 week and 1 day before your appointment (if applicable). Simply make note of your appointment and delete the emails.
Parents of patients under the age of 18, please request the appointment for yourself and enter “for (child’s name)” in the “reason for visit” section.
If you do not receive emails from Patient Fusion, please first review your spam filter, then call the office if it is not there.
Additional hours and home/workplace visits are available upon request. Dr. Blyskal is also available by cell phone, emailing and texting 7 days per week, 50 weeks per year (please see your patient contract for details).
